fix: resolve 9 high-severity maintenance audit findings (F-003 through F-012) (#668)

Protocol design drift (F-003, F-004, F-005):
- Add PEER_REQUEST/PEER_ACK message types to protocol-crate-design.md
- Add map_initial_data field to ProgramImage in S7.1
- Fix AeadProvider key param: &[u8] -> &[u8; 32] in S5.1

Test ID traceability (F-006, F-007):
- Add T-P010-P016 comments to AEAD codec inline tests
- Add T-P080-P089 comments to modem codec inline tests
- Add T-P100-P104 comments to BLE envelope inline tests

Node error message (F-008):
- Fix NodeError::AuthFailure: HMAC -> AEAD authentication failed

Node design drift (F-009):
- Update S2, S5, S16.2: ESP-IDF hardware AES -> RustCrypto aes-gcm

BLE pairing design (F-010):
- Merge S6.1-6.3 RETIRED sections into single consolidated block

BLE pairing validation (F-011):
- Collapse RETIRED test cases into compact references

Gateway logging tests (F-012):
- Implement T-1306a (file sink path derivation)
- Implement T-1306b (ETW provider - Windows cfg only)
- Implement T-1306c (runtime log-level reload)
