feat(vscode): add language icon for .ft tab and explorer

Wire contributes.languages.icon to light/dark SVGs so .ft files get a green
“FT” glyph in tabs and the Explorer when the file icon theme falls back to
language icons. Include icons/*.svg in packaged files for the VSIX.

Example (package.json):

    "icon": {
      "light": "./icons/forst-light.svg",
      "dark": "./icons/forst-dark.svg"
    }
