Add iTerm2 support via AppleScript (#30)

* feat: add iTerm2 support via osascript in launch-revdiff.sh

Split pane in the invoking session (found by UUID from $ITERM_SESSION_ID).
Auto-detect split direction from session dimensions (columns/rows).

* docs: update README and install.md for iTerm2 support

* fix: address review findings for iTerm2 support

Use launcher script with argv to avoid single-quote injection in paths.
Capture new session ID from split and close pane after revdiff exits.
Add iTerm2 block to launch-plan-review.sh for parity with other backends.
Rename AppleScript vars to avoid clash with iTerm2 property names.
Add README note about split view vs full overlay (API limitation).
Update SKILL.md description and terminal list to include iTerm2.

* fix: use ASCII >= operator in AppleScript for encoding safety
