utils.postprocess: resolve changelog boilerplate from GitHub releases

During post-processing, replace CHANGELOG.md Keep a Changelog and SemVer
intro URLs with the latest tags from olivierlacan/keep-a-changelog and
semver/semver when the CLI HTTP session is available; otherwise use pinned
fallbacks. Pass that session from main into post_process_steps. Cache only
the string URL helpers keyed by tag, not the async resolvers.
