fix: Require TIK uniqueness within a domain

The spec now requires that any TIK redeclaration within a domain either
share the exact same context and body, or be rejected at build time by
the TIK processor. Developers must disambiguate colliding occurrences
with distinct contexts rather than relying on foresight to add them
preemptively.
