feat(ci): add coveralls coverage reporting to backend and frontend (#78)

## Description

Integrates Coveralls into the backend and frontend CI workflows for
PR-level coverage delta reporting and enforces a minimum coverage
threshold as a required check.

### Added

- Coveralls upload step to backend and frontend workflows
- Coverage threshold enforcement (65%) with a workflow-level env var

### Updated

- `make test-backend` writes `coverage.out` locally instead of `/tmp/`
and no longer deletes it
- Jest config adds `json-summary` reporter for threshold parsing
- README testing section notes Testcontainers usage and Coveralls CI
integration

### Deleted

- CI/CD section from README (content folded into Testing section)
