fix: Run mutation tests file-by-file

This change modifies the mutation testing workflow to execute `muex` on
each changed Elixir file individually, rather than passing all changed
files at once. This allows for more granular feedback and prevents a
single failing test from blocking the entire workflow for unrelated
changes.

The `--fail-at` threshold has also been reduced to 20 for individual
files, and a timeout of 30000ms has been added to prevent excessively
long runs.
