feat(lsp): folding ranges, completion context, and formatting fixes

- Add folding range handler and advertise capability in initialize
- Improve completion (error position, keyword/context paths) and tests
- Fix formatting URI handling; extend formatting and server tests
- typechecker: run validateAssertionNode for ensure assertions; add infer_ensure tests
- docs: ROADMAP ensure `is` row; vscode-forst README; minor parser tweak
