Fix call graph to include method calls for cycle detection (#21)

The call graph only tracked plain function calls (*ast.Ident), missing
method calls (*ast.SelectorExpr). This caused false positives when
methods formed circular call chains. Also fix reachable() to use the
qualified caller key (TypeName.Method) instead of the short name.
