refactor: add MiddlewareFunc type and standardize config docs (#162)

- Add MiddlewareFunc type alias for func(http.Handler) http.Handler
- Update Router interface and implementation to use MiddlewareFunc consistently
- Standardize all middleware config comments to use "// Default:" format
- Add custom middleware example in examples/middleware/custom/
