Fix multi-value 'add more' fields (#416)

* Fix multi-value 'add more' fields

Delegate add/remove handlers in multivalueinputs.js (Bootstrap 5)

Add reusable _multi_value_field partial; update forms to use it

Remove legacy _form_multi_* partials and duplicate markup

* Move the 'add more' button back to the left side

* Update multi_value_spec tests to use new identifiers for buttons

* Refactor multivalueinputs.js for delegated events and stable row ids

Use document-level click delegation (data-field-name + js-* classes) so
add/remove work after Turbolinks navigation. Drop window.counts and
DOMContentLoaded button wiring. Compute next input id from max suffix + 1,
collect inputs by name, and guard double script inclusion.

* Align front/new.html.erb with qa except multi-value partials
Restore qa-style markup and helpers so the PR diff is limited to
replacing the three inline multi-value blocks with
software_records/multi_value_field renders.

* Drop shared multi_value_field; use qa-style form_multi_* + inline front/new
Remove _multi_value_field.html.erb. Software record form again renders the five
_form_multi_* partials (same role rules as qa, updated for delegated JS).
Request form inlines the three multi-value blocks like qa instead of rendering
the shared partial.
