feat(plugin): add smart ref detection for revdiff skill, bump to v0.2.0

Add detect-ref.sh script that infers the best diff ref based on branch
and uncommitted state. On feature branches with uncommitted changes,
the skill asks the user whether to review uncommitted only or full
branch diff against master.
