feat(embed): add Sapi trait, ServerContext, PhpThreadGuard and worker-mode lifecycle

Add high-level safe abstractions for building custom PHP SAPIs in Rust:

- `Sapi` trait with trampoline generation for type-safe SAPI callbacks
- `ServerContext` trait for per-request state management
- `PhpThreadGuard` RAII wrapper for ZTS thread lifecycle
- `ServerVarRegistrar` for safe $_SERVER population
- Worker-mode partial shutdown/startup for request recycling
- Guide documentation for Custom SAPI and Worker Mode
- Nix flake ZTS devshell
