Require home and namespace at node construction time, remove mutable setters (#47)

Strictify the API: cy_new() now takes (platform, home, namespace)
instead of just (platform). The home must be nonempty; namespace may be
empty. Both are normalized and embedded in the cy_t allocation (single
heap block, no separate free needed). cy_home_set/cy_namespace_set are
removed.

cy_udp_posix replaces cy_udp_posix_set_default_names with two pure
helpers: cy_udp_posix_home(base, prefix) and cy_udp_posix_namespace().
