Promote ClickHouse 'simple' ingestion schema logic (#3322)

* update query templates to only have the primary logic path with updated DDL

* update mapping config to use old "simple" configurations

* remove simple configuration option from clickhouse backend UI

* remove remaining "simple schema" path logic from clickhouse adaptor

* tighten up logic a bit
