fix: sort dump output chronologically; hide subagent transcripts by default

dump was printing entries file-by-file (parent then subagent), causing
subagent content to appear at the end rather than in chronological order.
This also meant dump|tail and tail showed different results.

Now dump collects all entries and sorts by timestamp, matching tail's
behavior. Subagent transcripts are hidden by default in both dump and
tail; pass --subagents to include them.
