* docs(communications): add external messaging integration patterns

- New guide for outbound/inbound Bots, conditional references, thread matching,
  webhook deduplication, and round-trip SMS routing
- Extend messaging-examples.ts with blocks for notifications, Twilio identifier
  thread setup, active-thread lookup, and round-trip reply handler
- Wire sidebar and communications index; QC fixes for links and SDK naming

- Replace outbound Subscription with create + executeBot for synchronous errors
- Link Consuming Webhooks and unauthenticated webhook guidance
- Upsert thread header by external conversation identifier
- Use searchOne for patient lookup in active-thread strategy
- Apply createResourceIfNoneExist across inbound/outbound examples
- Round-trip: persist reply then executeBot for round-trip bot (no Subscription)
