fix: use .ralphex/.gitignore instead of modifying root .gitignore (#260)

* use .ralphex/.gitignore instead of modifying root .gitignore

Replace EnsureIgnored/CommitIgnoreChanges with EnsureLocalGitignore
that creates a self-contained .ralphex/.gitignore with patterns for
runtime artifacts (progress/, worktrees/). The file also ignores itself
so it stays invisible to git status and doesn't interfere with branch
creation checks.

This eliminates root .gitignore modifications, the commit-after-ignore
logic, and ~270 lines of code.

* address review: remove isIgnored dead code, fix gosec warnings, update docs
