Docs/usage guide (#34)

* docs: add usage documentation with quickstart, guides, and assertions reference

New files in docs/guide/:
- quickstart.rst: end-to-end walkthrough with "How It Works" architecture explanation
- instructor-guide.rst: test cases, assertions, YAML serialization, runner.main(),
  nbgrader integration (pipeline, config, test cells, data generation workflow)
- student-guide.rst: reframed around pre-built locked cells, _i explanation
- assertions-reference.rst: all 13 assertion functions with signatures and examples

Modified:
- docs/usage.rst: rewritten as landing page with sub-toctree
- docs/index.rst: restructured into User Guide / Development / API Reference toctrees
