feat: add mapterhorn.com as default tile provider with configurable tile size and attribution

- Switch default tile URL to https://tiles.mapterhorn.com/{z}/{x}/{y}.webp
- Add tileSize to ElevationProviderConfig (default 512 for mapterhorn, validates power of 2)
- Add attribution to ElevationProviderConfig with mapterhorn defaults
- Change getAttribution() from static to instance method returning configured attribution
- Remove hardcoded TILE_SIZE=256; pass tileSize through ElevationCalculator to
  toPixel() and normalizePixel()
- Update README and CLAUDE.md to reflect new defaults and configurable attribution
