* chore: replace Codecov with Coveralls; fix CI badge URL

- Swap codecov/codecov-action for coverallsapp/github-action@v2
- Add simplecov-lcov dev dep; configure SimpleCov to emit combined
  coverage/lcov.info (report_with_single_file: true) alongside HTML
- Add ?branch=master to CI badge URL so it pins to master status
  and clears the stale "failing" state from the pre-polish initial push
