refactor: remove dead session chain-link detection code

The chain-link detection (read_line2, detect_chain_link, and the chain-
building logic in discover_sessions) never fires in practice — the
"[Request interrupted by user for tool use]" text never appears on
line 2 of any session file. Replace with a simple mtime sort.
