feat: add setupExtensions hook for custom express routes

Introduces a new setupExtensions configuration property to allow
injecting custom Express routes and middleware after the database
connection pools are initialized but before static file routes and
SPA fallbacks are mounted.

This enables native Node.js tasks (like IMAP processing, PDF
generation, etc.) to share the same Express application and Oracle
connection pools.

- Add setupExtensions callback to Zod config schema
- Execute hook in startServer passing Express app and DB pools
- Add unit tests for validation and server execution
- Update README.md with usage examples
