fix: replace showOpenFilePicker with input[type=file] to allow ~/.ssh/config

showOpenFilePicker blocks access to hidden/system directories on macOS.
Switch to a hidden <input type="file"> for both import and reload so that
~/.ssh/config can be opened without restriction.

- Remove sshConfigHandle state and showOpenFilePicker usage
- Add importModeRef to distinguish import (add-only) vs reload (upsert)
- Reload button now re-triggers the file input and appears after first import
