Update docs and install hints for extras and --user flag (#17)

## Summary

- All `pip install` commands now use `--user` flag across README,
skills, agent, and scripts
- Installation docs updated to show extras groups:
`karellen-lsp-mcp[clangd]`, `[jdtls]`, `[all]`
- LSP adapter install hints updated to reference extras instead of
direct package names
- pipx example uses quoted `'karellen-lsp-mcp[all]'` for shell safety

## Test plan

- [x] `pyb -vX` passes
- [x] All `pip install` references verified with `--user` flag
