refactor: externalize embedded templates using go:embed (#79)

- extract import block generation into templates/import_block.tmpl
- extract moved block generation into templates/moved_block.tmpl
- extract scaffold YAML rendering into templates/scaffold.yml.tmpl
- add buildScaffoldData to separate grouping logic from presentation
- template-based output ensures consistent 2-space YAML indentation
