fix: truncate lines by character count, not bytes

Use char_indices().nth(n) to convert char offsets to byte offsets,
so --max-line-width respects Unicode character width rather than raw
byte length. Add a regression test with 4-byte emoji that caught the
original byte-based truncation producing only 5 chars for max_width=20.
