Add per-tool timeout parameter and update documentation (#15)

## Summary

- **Per-tool `timeout` parameter**: All 21 MCP tools now accept an
optional `timeout` parameter (seconds) that overrides the daemon's
`LSP_MCP_READY_TIMEOUT` for that specific call. Defaults: 30s for
lifecycle tools, 120s for query tools.
- **Documentation updates**: README, CLAUDE.md, docs/c-cpp.md, skills,
agent, and MCP server instructions updated to document the timeout
parameter, `lsp_regenerate_index` tool, and 5% staleness threshold.
- **MCP server instructions**: Updated FastMCP instructions string to
mention `lsp_regenerate_index` and per-tool timeout.

## Test plan

- [x] Full `pyb -vX` build passes (unit tests, integration tests,
flake8, coverage)
- [x] Timeout parameter flows through: server.py → params dict →
daemon.py `_handle_lsp_request` → readiness waiting
- [x] Client-side timeout (`LSP_MCP_CLIENT_TIMEOUT`) remains untouched
as the outer safety net
