Code changes:
 - Phonemetadata has two more fields to represent possible lengths of phone
    numbers. Changed BuildMetadataFromXml to alter the way that metadata
    about possible-lengths information is consumed when constructing
    metadata to populate these.
 - Add BuildMetadataFromXmlTest
