refactor: improve internal code quality

- Pass options to super() in `ValidationError` for consistency with other errors classes
- Deduplicate `formatDecimalString` by keeping single source
- Validate signature length and normalize v=0/1 to v=27/28 in `splitSignature`
- Guard against empty addresses array in `getWalletAddress` for `ViemJsonRpcAccount`
- Export `ExchangeConfig` for symmetry with `InfoConfig` and `SubscriptionConfig` exports
- Add section separators to `_postRequest.ts` matching project convention
- Extract `L2BookLevel` helper type and add named tuple labels [bids, asks] to match info `L2BookResponse` structure
- Remove redundant `v.pipe(Integer, ...)` wrapper around `v.picklist`
- Remove redundant `| undefined` from optional fields
