fix: vault healthcheck and init script for uninitialized state

- healthcheck: vault status exits 2 when sealed/uninitialized; treat
  exit 2 as healthy (container running) and only exit 1 (connect error)
  as unhealthy
- vault-init-prod.sh: remove -f flag from curl so HTTP 501 (not
  initialized) does not cause infinite wait loop
