Init ratelimit: in-process per-key sliding window rate limiting with heap-based eviction

Uses heapx indexed min-heap to manage 1:1 map-heap node mapping.
Group lock is released before calling Limiter.Allow() to enable true
per-key concurrence. Configurable threshold and window duration.
