fix(node): log program install and chunk transfer errors (#469)

Spec (ND-1009):
- Add criteria 4-5: WARN log on program install failure and chunk
  transfer failure, including the error description.

Code (wake_cycle.rs):
- Both error paths in the UpdateProgram/RunEphemeral handler silently
  returned to sleep. Add \log::warn!()\ with the error so UART shows
  what actually failed.

Also removes accidentally committed pr-body.tmp.

Partial fix for #468.
