fix(otel): fix span leak on stream subscription cancellation

Replace StreamTransformer.fromHandlers with StreamController-based wrapper.
fromHandlers has no cancel callback, leaving spans open when consumers
unsubscribe early. New _wrapWithSpan uses onCancel to end the span in all
cases: normal completion, error, and cancellation. A finishOnce guard
prevents double span.end() if both onDone and onCancel fire.
