feat: add TestTerminalAdapter and refactor TerminalAdapter into a trait

Replace the monolithic TerminalBridge with a TerminalAdapter<B> trait
and per-backend adapter structs (Crossterm, Termion, Termwiz, Test).
The new TestTerminalAdapter wraps ratatui's TestBackend for integration
testing without requiring a real terminal.

Also fixes enter_alternate_screen error mapping in termwiz adapter
and updates all examples to use the trait's draw() method directly.

Closes #145
