Add comprehensive test coverage (174 tests, up from 8)

New test files:
- test_model_flags: flag_warn/flag_bad properties across 7 pipeline models
- test_model_paths: intermediate_output_path formatting
- test_spectrum_mixin: ivar-to-error conversion
- test_pipeline_from_spectrum: PipelineOutputMixin.from_spectrum
- test_base_helpers: get_fill_value, fits_column_kwargs, category_headers
- test_apogee_utils: _transform_err_to_ivar
- test_glossary: glossary attribute access and special contexts
- test_pipelines: pure helper functions across 7 pipeline modules

Expanded existing tests:
- test_utils: 1 -> 41 tests (version strings, expand_path, flatten, Timer)
- test_fields: 2 -> 16 tests (BitField, pixel array accessors)

Also: SqliteExtDatabase import fallback in base.py for peewee compat
