* Disable cohort duplication at 20

Enforce a hard limit of 20 cohorts and reflect that in the UI. Add a guard in cohortModalGenerator to prevent duplicating when there are already 20 cohorts. In the CohortList component compute isDuplicateDisabled (length >= 20), show the count as “x/20”, update the duplicate tooltip to indicate limit reached, disable the duplicate button and guard the click handler to avoid attempting duplication. These changes prevent creating more than 20 cohorts and provide clearer feedback to the user.

* Tweak cohort list styling and header count

Remove the hardcoded "/20" from the cohort list heading count and apply small UI tweaks: adjust header background color, increase action item gap to 16px, set action button padding to 0, and lower disabled opacity to 0.3. These changes refine spacing and the visual treatment of the cohort list items.
