fix: handle close error in history AppendToFile, clean up gitignore

AppendToFile now uses a named return with deferred close error capture
(matching the pattern in httpclient/client.go) instead of discarding
the f.Close() error. On write handles, Close() may flush buffered data
and a failure there means silent data loss.

Also expand .gitignore to cover prefixed coverage files (*_cov*.out,
*coverage*.out) and Claude Code worktrees (.claude/worktrees/).
