* feat(portal): Encounter detail page with vitals, diagnoses, and provider/patient cards

- Server Component reading encounter by ID via authenticated GraphQL
- Hero card: encounter type, status badge, date, chief complaint, notes, medical-report watermark
- Side-by-side Patient and Provider summary cards with avatar initials and deep links
- Vitals section: responsive 2-3 col grid with ekg-monitor icon header
- Diagnoses section: ICD-10 badge, status chip, diagnosis date, notes; shield-cross icon header
- Consistent status color coding (completed/scheduled/in_progress/cancelled/active/resolved/chronic)
