Add provider login UI and fix review diff state reset

Extend ProviderTestButtons to fetch provider metadata from the backend
via providers.list RPC, show configured/unconfigured status badges, and
add inline token input with login/update flow via provider.login RPC.
Fall back to a static provider list when the backend is unavailable.

Fix SimpleReviewSummary diff state clearing: move setDiffError(null) and
setFullDiff(null) from the useEffect into the click handler so stale
diff content is cleared immediately on file selection rather than on
every effect re-run.
