docs: refine e2e_usage.md for clarity and consistency

- Fix heading hierarchy (start with h1, consistent nesting)
- Fix section numbering (### 2.2 -> #### Windows under ### 2)
- Add descriptive subtitles (Dynamic/Static Provisioning)
- Fix Windows StorageClass example: use placeholder instead of
  svc.cluster.local which contradicts the warning above it
- Remove uid/gid mount options from Windows example (not applicable)
- Update Azure File link from docs.microsoft.com to learn.microsoft.com
- Improve grammar, punctuation, and formatting consistency
- Add horizontal rules between major sections for readability
