.claude: add changelog agent and update ci skill

Add changelog agent that analyses commits since the last tag and
updates CHANGELOG.md. Integrate it into the ci skill so the
changelog is updated before each commit. Add incidental files rule
so CHANGELOG.md and dictionary.txt do not affect the component
prefix unless committed alone.
