fix: use Content-Length for byte count instead of stream wrapping

The countingStream() async generator wrapper broke axios in
GatekeeperClient.addDataStream (axios serialized the generator as {}).
Instead, read Content-Length from the request header at the API layer
and pass it through as options.bytes. This keeps the stream as a native
Readable that axios handles natively.

Also adds unit tests for addDataStream/getDataStream across IPFS helia
client, gatekeeper CAS, gatekeeper client, and keymaster file stream
methods (13 new tests).
