feat: add IncludedPaths support to all middlewares (#128)

Add IncludedPaths field to all middleware configs, allowing users to
explicitly specify which paths a middleware should apply to (allow-list
approach) instead of just excluding paths (block-list approach).

- Added IncludedPaths to 19 config structs
- Added shouldProcessMiddleware() and validatePathConfig() helpers
- Updated all 17 middlewares to support both ExcludedPaths and IncludedPaths
- Standardized ExcludedPaths documentation across all configs
- Fixed tracer.go defaults to use []string{} for consistency
- Added comprehensive tests for path filtering in all config test files
