feat: code coverage for Next.js app (#11)

* feat: code coverage for Next.js app with @vitest/coverage-v8

- Install @vitest/coverage-v8 and configure v8 provider in vitest.config.ts
- Add html + lcov + text reporters and 80% thresholds across all metrics
- Exclude layout.tsx (font-loading infrastructure) from coverage
- Add tests for Home, Error, and GlobalError components — 100% coverage
- Add apps/ehr-portal/bin/coverage script (vitest run --coverage)
- Sync bin/steps/20_nextjs.sh to generate bin/coverage and install the provider

* feat: add Coveralls coverage badge

Replace the unconnected Codecov badge with Coveralls.
Update portal.yml to run vitest with --coverage and upload
the generated lcov.info to Coveralls via coverallsapp/github-action@v2.
