mcp: set Accept: application/json on upstream token exchange requests (#6195)

## Summary

RFC 6749 §4.1.3 and §5.1 require token endpoint responses to be JSON.
This PR makes that requirement explicit by setting the `Accept:
application/json` header on all upstream token exchange requests,
ensuring servers that perform content negotiation respond with JSON
rather than some internal defaults (i.e. GitHub would return
`application/x-www-form-urlencoded`).
