refactor(build): Switch ESM build to preserveModules for better tree-shaking (#4113)

- Switch Rollup ESM output from single flat file to preserveModules,
  enabling consumer bundlers to import only used modules
- Fix sideEffects: mark only CSS and UMD entry as side-effectful
- Update module/exports paths to dist-esm/index.esm.js
- Update tree-shaking test scenario import paths
