* fix: export missing helpers for rc.9 migration ergonomics

- Export makeKeyPackageRef() from keyPackage.ts root export
- Export getCredentialFromLeafIndex() from ratchetTree.ts root export
- Export canonical greaseValues constant from grease.ts
- Add regression tests for new root exports
- Update API report (ts-mls.api.md)

Fixes migration ergonomics regression where downstream libraries
could not access these helpers after rc.9 switched to root-only exports.

* test: remove rootExports test file
