Achieve 100% test coverage

- Remove dead code in initializeFile: the `if err != nil` check after
  Renamer (which has no error return) was unreachable
- Add nestedSubdirFailFS test helper and TestInitializePathRecursiveError
  to cover the error return in the recursive initializePath branch
- Add TestGenerateNilHasher to cover the cfg.Hasher == nil default branch
- Add TestPathedDirEntry to cover PathedDirEntry.IsDir, Type, and Info
