feat: Add settings tab and derive server URL at runtime (#209)

* feat: Add settings tab and derive server URL from hostname at runtime

Add Settings tab to gatekeeper-client and keymaster-client with server
URL configuration persisted in localStorage. Add server URL field to
keymaster-client LoginModal so users are never stuck if the URL is wrong.

Replace baked-in VITE_GATEKEEPER_URL/VITE_KEYMASTER_URL build args with
VITE_SERVER_PORT (just the port number). Clients now derive the full
server URL from window.location.hostname at runtime, so accessing via
VPN (e.g. http://extropy:4225) automatically targets http://extropy:4224.

* fix: Allow URL change in LoginModal without passphrase

Remove `required` from passphrase field and enable submit button when
the server URL has changed, so users can update the URL without needing
to enter a passphrase first.
